How It Works

Certintell’s JoinCare Team will be in direct contact with each enrolled patient and your clinic. These specialized care managers are behavior-change specialists committed to improving health outcomes through a proven process. They will identify patient health goals and barriers, create an achievable care plan of action and boost self-management confidence. Each month, a minimum of 20 minutes will be spent with each enrolled patient. A Medication Safety Review^TM is provided every six months.

The JoinCare Team continuously monitors new health data and will contact your clinic if they see anything high risk. Our goal is to facilitate interaction between your physicians and patients at home, while providing a clearer picture of a patient’s overall health at any given time. Ultimately, healthcare outcomes for adult patients are improved through increased control of high blood pressure, reduced proportion of obesity, reduced proportion of diabetes, decreased hospitalizations/readmissions and decreased medication safety-related problems.
