About Enhanced RPM
Enhanced RPM is an innovative project aimed at improving health outcomes in rural communities through remote patient monitoring (RPM) and comprehensive medication reviews. Through a collaborative effort, Enhanced RPM focuses on delivering primary care, remote patient monitoring, chronic care management and comprehensive medication management via telehealth. By participating in this program, healthcare providers are able to gain a better understanding of their patients’ overall health, while patient health outcomes and sustainability for health systems serving local communities are improved.
Research Project's Inclusion Criteria:
- Government Medicare (Part B), Dual-eligible beneficiaries, or only Iowa Medicaid
- One or more chronic diseases
- >10 meds or taking 1+ high risk medication
Remote Patient Monitoring by Certintell
Remote Patient Monitoring by Certintell provides connected health devices directly to the patient, allowing healthcare providers to monitor vitals regularly and take proper action when required. Monthly care management will be provided by Certintell’s JoinCare Team clinical unit, with the goal of improving health outcome measures, including, but not limited to, blood pressure, A1c, body weight and hospitalization/readmission rates.
Certintell is a care-management company delivering remote patient monitoring, chronic care management, behavioral health integration and transitional care management directly to patients as a virtual medical practice. These programs improve access to care, reduce the cost burden of chronically ill patients and increase revenues to the partnering health organizations, while improving overall clinical quality measures.
